Throughout our service engagement, Eternal Legacy AI "Company" may take photos and record audio and/or video of Participants, families, and service activities, for internal and external purposes including, but not limited to, archival purposes and for the Company's use in print, audio, video and electronic publications. This Photo, Video & Media Consent & Release Form "Consent & Release" permits the Company to use and publish a Participant's name, photo, audio, video or likeness in various Company publications to the extent and per the limitations set out in this form. External and internal Company publications include, but are not limited to, print and electronic marketing materials, the Company's official website and/or social media platforms (e.g., Instagram®, Facebook®, X®, LinkedIn®), training materials, research, annual reports, and promotional materials.

Permitted external use and publication of a Participant's name, identity, voice, audio, video or likeness will be limited to the consent provided per this release form. Participants/Guardians understand that the Company may have access to personal information about the Participant and family. Further, Participants/Guardians understand that a Participant's appearance in any Company marketing material inherently implies association with Eternal Legacy AI services.

 I authorize and give full consent to the Company to take, record, reproduce, use, edit, exhibit, display, broadcast, publish, distribute and create derivative works of audio and/or video recordings, photographs or images of the above-named Participant for use in external Company publications

The undersigned agrees to indemnify and hold harmless Eternal Legacy AI, its directors, officers, employees and agents from any claims arising from the Company's use of the Participant's photographs/images, audio/video recordings, identity, or likeness as discussed in this release form, to the greatest extent permitted by law.

The privacy and confidentiality of our Participants and their families is one of the Company's top priorities. Consent may be revoked at any time with written notification to the Company. However, revocation cannot affect materials already disclosed pursuant to this Consent & Release. This release form shall expire on 10/10/2028; however, expiration does not require the Company to remove materials already produced or published.

This Non-Disclosure Agreement "Agreement" is made as of 10/09/2025 by and between:

Disclosing Party: Eternal Legacy AI, Inc. 
Receiving Party: Form Applicant

1. Definition of Confidential Information  
For purposes of this Agreement, "Confidential Information" shall include all information disclosed by the Disclosing Party to the Receiving Party, whether in written, oral, or electronic form, that is designated as confidential or that should reasonably be understood to be confidential.

2. Obligations of Receiving Party 
The Receiving Party agrees to:  
a) Keep the Confidential Information confidential and not disclose it to any third party without the prior written consent of the Disclosing Party.  
b) Use the Confidential Information solely for the purpose of providing feedback on the Disclosing Party’s product.  
c) Take all reasonable precautions to protect the confidentiality of the Confidential Information.

3. Exclusions from Confidential Information  
Confidential Information does not include information that:  
a) Is or becomes publicly known through no breach of this Agreement by the Receiving Party.  
b) Is received from a third party without a breach of any obligation of confidentiality.  
c) Is independently developed by the Receiving Party without use of or reference to the Disclosing Party’s Confidential Information.

4. Term  
This Agreement shall commence on the date first written above and shall continue in effect for five (5) years, or until the Confidential Information no longer qualifies as confidential or until terminated by the Disclosing Party with thirty (30) days written notice.

5. No License  
Nothing in this Agreement grants any rights in or to the Confidential Information, except as expressly set forth herein.

6. Governing Law  
This Agreement shall be governed by and construed in accordance with the laws of the State of Delaware.
